*ALL SCHEDULED patients must call our office (914-769-0799) upon arrival. The front desk staff will let patients know when their provider is ready.

*Only scheduled patients will be permitted to enter the office.

*Adults requiring a caregiver or children 17 years and younger may present with one adult.

*ALL PATIENTS will be required to complete a COVID-19 screening form prior to being seen.
